CONSENT & RELEASE AGREEMENT

I hereby voluntarily, without consideration or any promise of any consideration, submit my written personal Network Care story/experience to the Global Gateway Foundation (“GGF”) for use by GGF, and/or any of its affiliated organizations, or any of its or their agents/agencies or representatives (“the GGF Affiliates”), for use as set forth in this Consent and Release. I agree and consent to allow GGF and/or any of the GGF Affiliates, and/or their assignees and/or licensees, to use, amend, transfer, display, publish, broadcast, reproduce and/or distribute publicly or otherwise any such written personal Network Care story/experience (hereinafter collectively referred to as "Individual Testimonial") for any purpose whatsoever, including, but not limited to, educational, promotional or commercial purposes, in any medium, as determined in the sole discretion of GGF or any of the GGF Affiliates. I also hereby agree and consent to the use of my name and/or any other information, including photographic still or video picture, which I submit, that may identify me, in connection with any use, distribution or publication of any such Individual Testimonial.

GGF and/or any of the GGF Affiliates may, at its or their sole discretion, make any changes in, additions to, or deletions from any such Individual Testimonial in their use thereof. Such alterations include, but are not limited to, cuts, edits, additions, deletions, changes, rearrangements, adaptations or language translations of the Individual Testimonial to different formats, and such other changes, additions and deletions as deemed necessary by GGF and/or any of the GGF Affiliates to make the Individual Testimonial viable for such use contemplated thereof. With reference to the alterations referred to above, I hereby waive any and all claims I may now or hereafter have to the rights of integrity, disclosure, attribution, withdrawal, retraction and reply, and any other rights that may be known as or referred to as "moral rights," as relate in any way to such Individual Testimonial and its use as contemplated herein.

GGF and/or any of the GGF Affiliates may use the Individual Testimonial worldwide and without limitation in time, and with or without stating my name, to the extent required by GGF and/or any of the GGF Affiliates as related to educational, sales, promotional or marketing purposes. Notwithstanding anything in this Consent and Release to the contrary, however, neither GGF nor any of the GGF Affiliates shall have any obligation to so use, display, publish, broadcast, reproduce or otherwise distribute any written personal Network Care story/experience submitted to it or them.

This Consent and Release contains the full terms of the release intended hereby and may not be changed except in writing signed by both GGF and me.

My submission of my Network Care story/experience/testimonial in any media to GGF shall serve as my agreement to the terms of the Consent & Release Agreement.
